What we do with your information

Who is responsible for your data

Forms Madrid, in Madrid, Spain, is the data controller for information submitted through this website. For any question about your data, or to exercise any right described below, write to mhogsett@formsacademy.com.

What we collect

We collect what you type into the application form, plus two technical details that help us reply properly.

From the form: the parent or guardian's name, email address and telephone number; the player's full name, birth year and the grade they are entering; the player's current club and level, if you tell us; the pathway you are interested in; whether the player is enrolled in Forms Academy Residency; anything you write in the message field; and your agreement to be contacted.

Automatically, alongside the form: which language version of the site you used, the page you submitted from, and the website that referred you, if any. We use these to reply in the right language and to understand which parts of the site families find useful.

We do not use analytics software, advertising pixels or tracking of any kind. This website sets no cookies.

Training bookings

If you ask for a training booking link at formsmadrid.com/train, we collect the parent or guardian's name, email and telephone number, the player's name and birth year, where you live, your travel arrangements, the dates you have in mind and anything you write to us. We use this to send your personal booking link and to follow up if you have not booked.

When you book, we also collect the player's date of birth, club, league level, division, position and stronger foot; any medical conditions, allergies or medication you choose to tell us about; an emergency contact; whether you agree to training photos; the dates and services you choose; and your signed acceptance of the Training Participation Agreement, with the time and the internet address it was signed from.

Payment is handled by Stripe. We never see your full card number; we receive only the payment confirmation and the country where the card was issued, which decides whether a card processing fee applies.

We process booking information to deliver the training you have booked (performance of a contract). We process medical information only because you give it to us to keep your child safe, which is your explicit consent; you can ask us to delete it at any time. Medical notes are deleted twelve months after the last booked session. Booking and payment records are kept for as long as Spanish tax and accounting law requires.

Information about children

The form asks for a player's name, birth year and school grade. We ask that a parent or legal guardian complete the form, not the player.

We collect this information to assess whether a program is an appropriate fit and to prepare an accurate quote, and for no other purpose. We do not build profiles of children, we do not share their information for marketing, and we never sell personal data.

Our legal basis

We process your information on the basis of your consent, which you give by ticking the box on the form. You may withdraw that consent at any time by writing to mhogsett@formsacademy.com, and withdrawing it does not affect anything done lawfully beforehand.

If you go on to become a Forms Madrid family, our further handling of your information is governed by the enrollment agreement and the performance of that contract.

Who else sees it

Application form submissions are stored and delivered by Netlify, the company that hosts this website, acting on our instructions. Training interest forms and bookings are stored in our database, hosted by Supabase; payments are processed by Stripe; and emails to you are sent through our email delivery provider. Each acts only on our instructions. It is then sent by email to mhogsett@formsacademy.com. Some of these providers are based in, or store data in, the United States; transfers there are covered by the European Commission's standard contractual clauses.

We do not sell personal data, and we do not share it with clubs, agents or intermediaries.

How long we keep it

If your inquiry does not lead to enrollment, we keep your submission for twenty-four months from our last contact with you, so we can pick the conversation up again if you return in a later season. After that we delete it.

If your player enrolls, records are kept for as long as the enrollment agreement and Spanish law require.

Your rights

Under the General Data Protection Regulation you may ask us to give you a copy of the information we hold about you, correct anything inaccurate, delete it, restrict what we do with it, provide it in a portable format, or stop processing it altogether. You may also withdraw consent at any time.

Write to mhogsett@formsacademy.com and we will respond within one month. If you believe we have handled your information badly, you may complain to the Agencia Española de Protección de Datos at www.aepd.es.

Changes to this page

If we change how we handle personal information, we will update this page and change the date at the top.
