What we do with your information

Forms Madrid runs this website and the application form on it. This page explains what we collect, why, how long we keep it, and what you can ask us to do with it.

Who is responsible for your data

Forms Madrid, in Madrid, Spain, is the data controller for information submitted through this website. For any question about your data, or to exercise any right described below, write to mhogsett@formsacademy.com.

What we collect

We collect what you type into the application form, plus two technical details that help us reply properly.

From the form: the parent or guardian's name, email address and telephone number; the player's full name, birth year and the grade they are entering; the player's current club and level, if you tell us; the pathway you are interested in; whether the player is enrolled in Forms Academy Residency; anything you write in the message field; and your agreement to be contacted.

Automatically, alongside the form: which language version of the site you used, the page you submitted from, and the website that referred you, if any. We use these to reply in the right language and to understand which parts of the site families find useful.

We do not use analytics software, advertising pixels or tracking of any kind. This website sets no cookies.

Information about children

The form asks for a player's name, birth year and school grade. We ask that a parent or legal guardian complete the form, not the player.

We collect this information to assess whether a program is an appropriate fit and to prepare an accurate quote, and for no other purpose. We do not build profiles of children, we do not share their information for marketing, and we never sell personal data.

Our legal basis

We process your information on the basis of your consent, which you give by ticking the box on the form. You may withdraw that consent at any time by writing to mhogsett@formsacademy.com, and withdrawing it does not affect anything done lawfully beforehand.

If you go on to become a Forms Madrid family, our further handling of your information is governed by the enrollment agreement and the performance of that contract.

Who else sees it

Your submission is stored and delivered by Netlify, the company that hosts this website, acting on our instructions. It is then sent by email to mhogsett@formsacademy.com. Netlify is based in the United States, and transfers of personal data there are covered by the European Commission's standard contractual clauses.

We do not sell personal data, and we do not share it with clubs, agents or intermediaries.

How long we keep it

If your inquiry does not lead to enrollment, we keep your submission for twenty-four months from our last contact with you, so we can pick the conversation up again if you return in a later season. After that we delete it.

If your player enrolls, records are kept for as long as the enrollment agreement and Spanish law require.

Your rights

Under the General Data Protection Regulation you may ask us to give you a copy of the information we hold about you, correct anything inaccurate, delete it, restrict what we do with it, provide it in a portable format, or stop processing it altogether. You may also withdraw consent at any time.

Write to mhogsett@formsacademy.com and we will respond within one month. If you believe we have handled your information badly, you may complain to the Agencia Española de Protección de Datos at www.aepd.es.

Changes to this page

If we change how we handle personal information, we will update this page and change the date at the top.
